About the Role
We’re seeking a dedicated Service Coordinator to help deliver a responsive, community‑based home support service for older people. In this role, you’ll work closely with a multidisciplinary team to ensure care is tailored, flexible, and aligned with each person’s goals for living well at home.
This is a 12‑month fixed‑term position covering maternity leave.
Responsibilities
- Support the multidisciplinary team to deliver high‑quality community support
- Complete clinical assessments for non‑complex clients
- Build strong relationships with clients, whanau, and health professionals
About You
- Registered health professional (RN, OT, Physio) with current NZ practicing certificate
- Minimum three years post-registration experience in aged care or community health
- Demonstrated ability to develop person-centered support plans
